Fail Safety

During Operation it is very common for parts to fail because of wear fatigue or overheating and in some cases catastrophic failure. Good maintenance procedure can reduce these activities to a minimum. It is important to note that in some cases minor failures can produce a catastrophic event such as "Piper Alpha" or the blow out in the Gulf of Mexico. Safety engineers will normally carry out an analysis of a piece of equipment or assembly such as "failure mode analysis" in order to improve the engineering and control systems for the client operators.
